In 2001, the WHO recognized depressive disorders as the leading cause of disability worldwide. But most Americans who meet diagnostic criteria for major depression are untreated or undertreated. Luckily, recent advances have finally made it possible for the field of public health to address mental health in the population. Public Health Perspectives on Depressive Disorders fills a gap by identifying the tools and strategies of public health practice and by exploring their application to twenty-first-century public mental health policy and practice. By looking at depressive disorders through a public health lens, this book highlights the centrality of mental health to public health. Linking the available research on depressive illness at the population level with public mental health policy and practice, expert contributors set a research agenda that will help make mental health a central part of public health science and practice. This book is an invaluable resource for researchers and practitioners to develop, facilitate, and conduct pilot and feasibility studies of promising preventive and treatment interventions that might mitigate the progression toward major depression and other mental disorders among populations at risk. The first part of the book underscores the public health significance of depressive illness by focusing on the evidence provided by recent approaches to nosology, epidemiology, illness burden, and impact on overall health. The second part looks at the social and environmental influences on depressive disorders that are critical to future efforts to prevent illness and to promote mentally healthy communities. About one American in five receives a diagnosis of major depression over the course of a lifetime. That's despite the fact that many such patients have no mood disorder; they're not sad, but suffer from anxiety, fatigue, insomnia, or a tendency to obsess about the whole business. |There is a term for what they have,| writes Edward Shorter, |and it's a good old-fashioned term that has gone out of use. They have nerves.| In How Everyone Became Depressed, Edward Shorter, a distinguished professor of psychiatry and the history of medicine argues for a return to the old fashioned concept of nervous illness. These are, he writes, diseases of the entire body, not the mind, and as was recognized as early as the 1600s. Shorter traces the evolution of the concept of |nerves| and the |nervous breakdown| in western medical thought. He points to a great paradigm shift in the first third of the twentieth century, driven especially by Freud, that transferred behavioral disorders from neurology to psychiatry, spotlighting the mind, not the body. The catch-all term |depression| now applies to virtually everything, |a jumble of non-disease entities, created by political infighting within psychiatry, by competitive struggles in the pharmaceutical industry, and by the whimsy of the regulators.| Depression is a real and very serious illness, he argues; it should not be diagnosed so promiscuously, and certainly not without regard to the rest of the body. Meloncholia, he writes, |the quintessence of the nervous breakdown, reaches deep into the endocrine system, which governs the thyroid and adrenal glands among other organs.| In a learned yet provocative challenge to psychiatry, Shorter argues that the continuing misuse of |depression| represents nothing less than |the failure of the scientific imagination.|

William Cullen (1710-1790) was a key figure in the Scottish Enlightenment. He was a friend of Adam Smith and David Hume but his achievements have not been so well recognised. Cullen was a great teacher who explored the relationship between medicine and science in a spirit of liberal inquiry and he played a major role in establishing the Edinburgh of his day as the world's foremost medical centre. He attracted many students from overseas, and the medical schools and institutions founded by his pupils set the pattern for medicine in North America. Cullen was born in Hamilton and educated at Glasgow, where he became Professor of Medicine before moving to Edinburgh to take up a sequence of professorships at the University. Like a true lad o' pairts he became First Physician to the King in Scotland and President of the Royal College of Physicians of Edinburgh. In addition he was a founder member of the Royal Society of Edinburgh. This book follows an exhibition and a symposium held to commemorate the 200th anniversary of Cullen's death and restores Cullen to his rightful place in both the Scottish Enlightenment and the history of medicine and science.
